How to Schedule

Insurance

Dr. Likhari accepts Medicare and is in-network for most major commercial insurance plans. Please contact your insurance company to determine whether Dr. Likhari is in-network for your specific plan.

Most insurance plans have deductibles, copays, and coinsurance provisions. Patients are expected to pay these fees at the time of the visit. If you have a high deductible health plan and you have not met the deductible for your insurance plan, you will be expected to pay the full fee at the time of your visit.

Out of Network:

If Sweetwater Dermatology is not in-network with your insurance, you are welcome to be seen as a patient. Fees are due and payable at the time of your visit. We will issue you an insurance-coded, itemized receipt that you may file with your insurer for reimbursement under the terms of your insurance plan.

Patients without Insurance:

Sweetwater Dermatology welcomes individuals who do not have insurance. We will provide you with a self-pay fee estimate. Fees are due and payable at the time of your visit.

Before Your Appointment:

Once you schedule an appointment, you will receive an email asking you to sign up for the Sadio Patient Portal. It is recommended that you sign up ahead of time and fill out your new patient paperwork and consent forms. Alternatively, you can do this when you check in, but please arrive 15 minutes before your scheduled appointment to allow enough time.

Directions:

We are located at 3533 Town Center Blvd S. inside the Sugar Land Med-Ped Clinic building. This building is at the corner of Sweetwater Blvd and Town Center Blvd., behind the Methodist Hospital. Suite 200 is on your right as soon as you enter the waiting room.

What To Bring:

If you have Medicare or an insurance plan, please bring your insurance card and proof of identity such as a driver’s license. It is very helpful if you write down or bring all medications and supplements (vitamins, health products) that you are taking, and any products you are using on your skin. If relevant, please contact your former health care provider and request that your records be forwarded to our office. Procedures:

If you have an excision scheduled, please remember to discontinue any medications prior to your procedure that Dr. Likhari might have discussed with you. If you are having something benign removed, like a cyst or lipoma, please make sure that you have called your insurance company with the billing codes provided to you at your consultation and are aware of any out-of-pocket fees. All fees are due at the time of service. Please arrive 30 minutes prior to your scheduled time so we have ample time to do a preoperative evaluation.

Cancellations:

If you are unable to make an appointment, please let us know as soon as possible and no later than 24 hours prior to the time of the appointment. Patients who no-show or cancel with less than 24 hours’ notice are subject to a $50 fee.

Text Messages:

Our office handles a lot of communication via text message, including appointment reminders and insurance card verification. By scheduling with us, you are consenting to receive text message communication, unless you express to us in writing that you would like to opt-out.
